To schedule your virtual consultation, please fill out the form below. Dr. Wise will email you with a HIPPA Compliant link to connect virtually.

NOTICE: Please sign & return all intake forms 48 hours in advance before your scheduled initial appointment.

Once all forms are received by Dr. Wise, she will send you a confirmation email with a HIPPA compliant link to join your virtual session. Please follow the instructions in that email to secure your appointment.

DO NOT USE ONLINE/SOCIAL MEDIA SITES TO CONTACT DR. WISE FOR EMERGENCIES OR MEDICAL COMMUNICATIONS.
